First Visit

A patient’s initial appointment can consist of a consultation explaining the diagnosis and treatment options but we often perform surgery on the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation, and a follow-up appointment may be needed to provide treatment.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any x-rays, if applicable.
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. Completing this paperwork in advance saves time and allows us to help you process claims faster and more effectively.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

A pre-operative consultation and physical examination are mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours before surgery. You will also need an adult to drive you home.

Please alert the office if there is any medical condition that may be of concern before surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever) or if taking any medications (i.e., heart medications, aspirin, anticoagulant therapy).

X-Rays

If the dentist or physician has taken x-rays recently, please have them forward them to our office. If unable to, please bring a copy to the appointment. We do have the capability to capture new images at our office if need be.
